Below are links to all of the forms required for new students at the OpenMinds Language Immersion Preschool.

The Personal Rights form needs the following information for compleation:

A Copy of your Child's Immunization Records also Required

Turn these forms in prior to your student’s start date at OpenMinds Language Immersion Preschool.  Students cannot begin school until all paperwork is turned in.

Forms may be uploaded directly to your child’s Sandbox profile, or you can drop a hard copy off in the mail slot at our studio/office location at 4236 Adams Ave, San Diego, CA 92116.

Once we have your student’s paperwork we will review it for any missing items, contact you with any questions and then create the file for your student so everything is ready for their first day of school with OpenMinds Language Immersion Preschool.
